SERVICES AND RECIPIENTS

  1. Total number of children and youth served by Carlton Manor Inc.: 178
  2. Increase/Decrease in total number served in 2012-2013 as compared with the 155 children and youth served in 2011-2012: +23
  3. Number served in Speciapzed Therapeutic Group Home I Program: 24
  4. Number served in Speciapzed Therapeutic Group Home II Program: 27
  5. Number served in the Enhanced Care Group Home Program (ECP): 21
  6. Number served in the Supported Foster Care Program (SFC) 106

TOTAL AGENCY OUTCOMES

A. Carlton Manor's Children's Mental Health Services data which is based on the Department of Children and Families Mental Health Outcomes: A total of 175 Seriously Emotionally Disturbed and Emotionally Disturbed (ED) children and youth were served.

B. The average annual days that all children and youth served by Carlton Manor Inc. programs were successfully served and maintained in a community based, less restrictive placement in 2012-2013 by program is: CMI 100%; CMII 99%; CMIII 100% and SFC 100%. The DCF State outcome target for the services provided is 96% or 350 days per year. All Carlton Manor Inc. programs continued to exceed the State-Wide DCF target objective for number of days children and youth are successfully served in the community in FY 2012-2013.

C. The percent of school days successfully attended by children and youth served by Carlton Manor Inc. by program is: CMI 97%; CMII 95%; CMIII 97% and SFC 96%. The DCF State target for the services provided is 89.8%. All Carlton Manor Inc. programs continued to exceed the State-Wide DCF target objective for successful school attendance in FY 2012-2013.

D. The number of children and youth successfully completing the program as compared to the number of children and youth unsuccessful at discharge is as follows. CMI 14 children and youth successfully discharged 100%; CMII 17 of 21 children and youth successfully discharged 81%; CMIII 11 children and youth successfully discharged 100%; SFC 41 of 43 children and youth successfully discharged 95%.
